Doctors and dentists
Once you have referred to the section “reasons why people visit Haydom” you may wish to apply for a visit. The following documents are necessary to support permit applications for all doctors/dentists who will work in the clinical area at Haydom.
- Curriculum Vitae
This does not need to be in great detail, and should deal mainly with your professional activities. Please note that if you mention a qualification in your CV, then you will have to produce a certificate to support it.
- 8 passport photos. (You might bring an extra set with you also, as they are sometimes needed, and it is easier to get them in your home country.)
- TWO CERTIFIED copies of each the following
- Professional certificates (degrees, diplomas etc)
- these are the certificate of your FIRST DEGREE, and any certificate of a SPECIALIST QUALIFICATION
- Evidence of medical/dental registration in the home country, and Original Letter of good standing with their own medical council
- This is documentation showing that you are currently registered as a doctor/dentist in the country where you work, and that they have no problems with you.
- Suitable documents would be from UK General Medical Council, Norwegian Medical Association, Norwegian Health and Social Security Directorate
- Passport (copy of ALL pages, also blank pages)
What do we mean by certified copies?
These are photocopies of your documents, which have been presented to a lawyer or certified registrar together with the original certificate, document or diploma. The lawyer/registrar then signs the photocopy, and writes on it declaring that this is a true copy of the original. He will then place his stamp on the document.
If your certificate is not in English:
Applicants must take note that any certificates and documents provided should be in English or accompanied by a certified correct English translation (2 copies also)
Application forms:
We can send you application forms by email attachment. Please ask us for copies.
Post your documents to us by normal mail, or get a friend to bring them.
References:
We request from you the names of 2-3 referees, including the present employer
Medical Defence Insurance:
We also request that you discuss your plans with your current medical defence insurance company to see if they will extend your cover to the time at Haydom.
